The Adolescent Day Treatment Program serves children 12-18 years of age. These children are placed in the program to address significant social, emotional and behavioral disorders. Educational services to these students are provided by Northwest Regional ESD teachers, under the supervision of a NWRESD principal. Therapeutic services are provided by Lifeworks Northwest treatment staff, under the supervision of a Lifeworks NW program director. The complex array of needs of the children and the families are met through the ongoing collaboration between the two providing entities.

Please complete this referral form to refer students to the day treatment programs or to REACH. Please complete all sections of the form and ATTACH ALL REQUIRED MATERIALS. We need all the required attachments before the student enrolls in our programs to serve them appropriately. This form and attachments need to be submitted regardless of whether the referral originates with a school district or with a county agency.
